I have been divorced for six months, but i would like to know how i can change my last name to my maiden name.

Take your official divorce decree down to the Social Security office and it takes just a few minutes for them to change it and send you a new card. Then take the divorce decree down to the Secretary of State's office and they'll change your driver's license. Once you get your new SS card and drivers license, you can go to the bank and change the name on your bank account (they usually require the SS card as proof that you've legally changed it so that you don't get in trouble with the IRS).
After that, it's only a matter of notifying your creditors that your surname has changed and what the new name is. If they ask for proof, you just let them see your new driver's license, but you don't have to send them anything special.
Reply:it should have been in the divorde decree. using your divorce papers go to the social security office get a new card in your madien name then go from there drivers license, bank etc....
Reply:You can fill out a "change of name" form for Social Security, just like you did when you got married. It's exactly the same process.
